Whitening Root-Canal-Treated Teeth: The Walking Bleach Technique
Why a root-canal-treated tooth darkens, how the walking bleach technique works, and why the thermocatalytic technique using heat has been progressively abandoned.
A tooth that has undergone root canal treatment tends, over time, to darken relative to neighboring teeth, taking on shades ranging from gray to brown. The most common cause is the presence of residual necrotic pulp tissue or blood-breakdown products left in the pulp chamber, which penetrate the dentinal tubules and alter light transmission through the tooth. Before proceeding with any internal whitening technique, it's always necessary to verify that the root canal treatment is adequate: if it isn't, it should be redone before addressing color.
The most widely used technique for whitening non-vital teeth is the so-called walking bleach, first described in its modern form in the early 1960s: after removing gutta-percha from the pulp chamber down to a level just below the cemento-enamel junction (to protect the underlying canal filling), a whitening agent — carbamide peroxide, hydrogen peroxide, or a mixture of sodium perborate and hydrogen peroxide — is placed inside the chamber and then temporarily sealed within the tooth. The patient continues normal daily activities (hence the name "walking bleach") while the agent acts from within; the whitening agent is typically replaced every 3-7 days, and generally 2 to 4 sessions are needed for a satisfactory result.
A more recent variant, described by Haywood and DiAngelis in 2010, is the closed "inside/outside" technique: after sealing the whitening agent inside the cavity, the patient also wears a single-tooth tray with whitening gel overnight, combining internal and external action until the desired shade is reached. In cases where walking bleach doesn't produce satisfactory results after a few applications, it can be combined with or replaced by an external (in-office) technique applied directly to the surface of the non-vital tooth.
A technique historically used but now generally discouraged is the thermocatalytic technique, which involved activating hydrogen peroxide with a heat source applied directly to the tooth. Combining heat with a whitening agent inside the pulp chamber has been progressively abandoned in modern clinical practice due to the risk of post-whitening complications, particularly external cervical resorption — a rare but serious complication that has led the dental community to prefer room-temperature techniques such as classic walking bleach.
