Nitrile gloves: what the AQL figure actually means and why double gloving is not always safer

Why nitrile has replaced latex in most practices, what the AQL stated on the box measures, and why removal technique matters more than glove quality.

The shift from latex to nitrile happened for two distinct reasons worth keeping apart. The first is allergy: natural latex proteins are a known allergen for both staff and patients, with reactions ranging from contact dermatitis to systemic presentations, and nitrile is a synthetic polymer that does not contain those proteins. The second is mechanical: nitrile resists many substances used daily in practice better — resin monomers, disinfectants, oils — and above all has a characteristic latex lacks, namely a tendency to tear visibly rather than perforate in pinpoint fashion. A glove that tears is seen and changed; a glove that punctures without a mark continues to be worn in the belief that it is intact.

The AQL printed on the box is the most misunderstood technical figure. It is not an index of general quality: it is the acceptable quality level for holes, that is the maximum percentage of defective gloves tolerated in the batch under the sampling prescribed by the standard. An AQL of 1.5 means up to 1.5% of gloves may have a micro-perforation and the batch still complies; an AQL of 1.0 is stricter. The practical consequence is counterintuitive: no AQL guarantees that the individual glove you are wearing is intact, and this is why recommendations on changing at regular intervals during long procedures exist regardless of the stated quality.

On thickness the choice is a real trade-off. A thicker glove protects more and punctures less, but reduces tactile sensitivity — which in dentistry is no small matter, because perceiving a root surface or the margin of a preparation runs through the fingertips. The sensible answer is not one thickness for everything but differentiation: thinner gloves for restorative and diagnostic procedures, thicker ones for surgery and for instrument reprocessing, where sensitivity matters little and puncture risk is highest. Powder-free gloves are now the standard too: lubricating powder carries allergens, settles on surfaces and interferes with the bonding of composite materials.

Finally, the point where most of the benefit is lost: removal. A glove pulled off by the fingers contaminates the hands with what was on the outside, undoing the protection of the whole appointment. The correct sequence is to pinch the cuff of the first glove from the outside, peel it off inside out, hold it in the palm of the still-gloved hand, slide a bare finger under the cuff of the second from the inside and peel that off over the first. It takes three seconds, and it separates a barrier that worked from one that merely gave the impression of working. Hand washing remains necessary afterwards, not instead.
