Disposable bibs: what makes a barrier effective and why the number of layers matters

How a dental bib is constructed, why the waterproof layer does the actual work, and how to correctly dispose of an item that has received aerosol for an entire appointment.

A professional dental bib is not a sheet of paper: it is a laminate, typically three-ply, and each layer has a different function. The two outer paper layers absorb the liquids falling during the appointment — coolant water, saliva, rinsing solutions — while the intermediate polyethylene layer is what stops those liquids passing through the material and reaching the patient's clothing. A bib without a waterproof layer absorbs and then lets through: it gives the impression of working for the first few minutes and fails exactly when needed, that is during scaling or preparation with the turbine, which are the moments producing most liquid.

Weight and dimensions determine the rest. A short bib leaves uncovered the part of the chest where liquid actually falls when the patient is supine, which is not the area immediately below the chin; a low weight tears when the clip tensions it, and a bib torn mid-appointment has to be replaced with already-contaminated hands. These are two details noticed only in use, and they are the reason it is worth evaluating a product over a full pack before ordering a year's supply.

In infection-control terms, the bib belongs to the same chain as the clip holding it: throughout the appointment it receives the aerosol produced by turbine and scaler, that is the suspension of saliva, blood and water that settles on every nearby surface. From this follows a simple but often neglected operating rule: it is removed by rolling it in on itself, contaminated face inward, and before taking off the gloves. Removing it open disperses into the air what had settled on it, which is exactly the opposite of what the item was there to do.
