The Link Between Oral Health and Systemic Health

In recent decades the scientific literature has accumulated consistent evidence for a principle that redefines the dentist's role: mouth health isn't isolated from the rest of the body. Periodontitis, in particular, has been called by some authors the "sixth complication of diabetes", alongside retinopathy, nephropathy, neuropathy, cardiovascular disease and wound-healing problems — a definition reflecting the now well-documented bidirectional relationship between the two conditions.

This relationship works in both directions: uncontrolled diabetes worsens the severity and progression of periodontitis, altering the host's immune response to pathogenic bacteria; at the same time, chronic inflammation of periodontal origin can worsen glycemic control, contributing to a vicious cycle that makes managing both conditions harder if treated separately.

A second area with growing evidence concerns the cardiovascular system. In 2020 the European Federation of Periodontology and the World Heart Federation published a joint consensus document recognizing an association between periodontitis and cardiovascular disease, mediated by mechanisms such as transient bacteremia (oral bacteria entering the bloodstream during chewing or dental procedures) and chronic low-grade systemic inflammation, which contributes to atherosclerotic processes.

For the clinician, this translates into a practical responsibility: taking a thorough medical history including diabetes, cardiovascular disease and other relevant systemic conditions, communicating to the patient the link between oral hygiene and overall health as an additional motivational point, and in some cases coordinating with the patient's physician for genuinely integrated management, especially in patients with severe periodontitis and significant systemic comorbidities.
