Diet and Dental Health: What to Recommend to Patients

A useful way to communicate the relationship between diet and dental health to patients is classifying foods into three categories based on their behavior in the presence of bacterial plaque. Cariogenic foods — simple sugars, refined grains, soft drinks and industrial fruit juices — promote caries onset by providing fermentable substrate to cariogenic bacteria. Cariostatic foods, such as meat, fish, fats and most vegetables, have no direct effect on caries. Anticariogenic foods, finally — fresh low-sugar fruit, vegetables, milk and aged cheeses — reduce caries risk, in some cases by stimulating salivary flow or directly countering the activity of acid-producing bacteria.
A point worth explicitly clarifying to patients is that sugar intake frequency matters at least as much as total amount consumed. Every time fermentable sugars are ingested, plaque pH drops for roughly 20-30 minutes before returning to baseline levels; consuming small amounts of sugar five times a day generates more acid-attack episodes than a single larger portion consumed during a main meal. This is why the most effective practical recommendation isn't so much "eliminate sugar" as "consume sugars during main meals, not as isolated snacks between meals".
Dental erosion, a process distinct from caries, deserves separate mention: it's caused by the direct chemical action of acids (citrus fruit, soft drinks, wine, gastroesophageal reflux) on enamel, regardless of the presence of bacterial plaque or cariogenic bacteria. For patients with frequent consumption of acidic foods or drinks, it's useful to advise against brushing teeth immediately after intake — enamel temporarily softened by acid is more vulnerable to mechanical abrasion from the toothbrush — but to wait at least 20-30 minutes, possibly rinsing the mouth with water in the meantime.
Among sugars, xylitol deserves specific mention for its different behavior compared to other sweeteners: while having sweetening power comparable to sucrose, it isn't fermented by cariogenic bacteria in the same way, and some studies credit it with directly reducing Streptococcus mutans concentration in the oral cavity. It's commonly found in sugar-free chewing gum and candies specifically marketed for caries prevention.
