Interdental measuring gauge: how much enamel can really be removed with stripping

Why stripping performed by feel is the quickest way to exceed safe limits, how much space is realistically gained per arch, and how to measure a black triangle before deciding whether to close it.

Interproximal enamel reduction is a conservative and predictable procedure as long as it stays within measurable limits, and becomes a problem when the amount removed is estimated by eye. Interproximal enamel thickness is not uniform: on lower incisors it is thin and leaves narrow margins, on posteriors it is more generous. The commonly shared operating rule is not to exceed roughly half the available thickness per surface, which translates into a few tenths of a millimetre per side and a realistic gain of a few millimetres per arch — not the figures sometimes quoted. Measuring means knowing when to stop; not measuring means finding out when the patient's sensitivity reports it.

The interdental gauge, in its commonest form a calibrated blade with graduated thicknesses, serves exactly this purpose: it is inserted into the interproximal space before and after reduction, and the difference between the thickness that passes at the start and the one that passes at the end is the amount actually removed. It is an objective check that replaces estimation, and it has a second advantage: it allows the reduction to be distributed across several contacts rather than concentrated on one, which is what almost always happens when working without control.

The same instrument is useful in assessing black triangles. Deciding whether to close an interproximal space with enamel reduction, with composite restoration or by leaving it alone depends on two measurements: the width of the space and the distance between the contact point and the bone crest. Beyond five millimetres of distance the papilla will rarely fill the space on its own, and persisting with stripping to bring the teeth closer produces a long, unaesthetic contact without solving the problem. Here too, measurement guides the therapeutic choice better than visual impression, which tends to overestimate spaces anteriorly and underestimate them posteriorly.

In practice, reduction should always be followed by polishing and fluoride application: an enamel surface left rough retains plaque and increases the risk of interproximal caries, which is the complication that makes stripping a contested procedure when it is performed badly.
