Buccal tube tweezers: why a positioning error on the molar is paid for at the end of treatment

How a buccal tube is transferred into the correct position, why one millimetre of error on the molar does more damage than on an incisor, and what changes between direct and indirect bonding.

The buccal tube is the distal anchorage of the archwire, and its position determines the inclination and rotation of the molar for the whole of treatment. A positioning error on an incisor is corrected at the finishing stage with a bend; the same error on a molar propagates along the entire arch, because the molar is where the mechanics discharge the greatest forces and every angular deviation is amplified on the anterior teeth. That is why it is worth spending more time positioning tubes than bonding anterior brackets, counter to intuition — which tends to focus attention on the visible zone.

Dedicated tweezers solve three problems at once. They hold a small, slippery attachment already loaded with adhesive without it rotating between the beaks, which a general-purpose tweezer does not guarantee. They allow the buccal surface of the molar to be reached while keeping the forearm in a natural position, thanks to beak angulation designed for the posterior sector. And they permit clean release of the attachment once positioned, without dragging it: dragging at the moment of release is the commonest cause of attachments ending up a millimetre out of place relative to where they were set down.

In positioning, the reference is the clinical long axis of the crown, and height should be measured from the mesiobuccal cusp, not judged by eye against the gingival margin, which is variable and often asymmetrical between right and left. A second point to verify before curing is interference with the antagonist: a tube placed too occlusally becomes a premature contact the patient reports at the first meal, and which often resolves only by debonding and repositioning. Indirect bonding, with a transfer tray built on a model, reduces both problems and finds its strongest indication precisely in the posterior sectors, where direct vision is poorest.
