Ultrasonics or curettes: they are not alternatives, they are different stages
The choice between ultrasonic and hand instrumentation is framed as an alternative, and it is largely a false one: the two systems do different things and perform better together.
The ultrasonic works by high-frequency vibration, which shatters deposits, accompanied by cavitation and acoustic streaming of the coolant.
That streaming has an effect of its own: it disrupts biofilm even where the tip does not physically reach, which is why ultrasonics are particularly effective in deep pockets and furcations.
The curette works by mechanical cutting, and has an advantage the ultrasonic does not offer: tactile feedback. The operator feels the root surface and recognises when it is smooth.
This makes it irreplaceable in the finishing stage and in verification, because smoothness is judged with the instrument more than with the eye.
The sequence giving the best results is therefore ordered: ultrasonics to remove the bulk of the deposits, curettes for finishing and checking the surfaces.
Using curettes alone lengthens appointments and tires the operator; using ultrasonics alone leaves surfaces that still feel irregular and that never get verified.
On settings, power should be kept as low as remains effective. High power does not increase efficacy proportionally and increases removal of root tissue.
Angulation matters as much as power: the tip is kept parallel to the root surface, never perpendicular, and in constant motion without dwelling on one point.
Cooling is not an accessory. Without adequate flow the tip generates enough heat to damage the pulp, and the flow is also what produces the disruptive effect.
Sharpening curettes is the most frequently neglected factor. An unsharpened curette does not cut but burnishes the deposit, making it harder to remove and lengthening the appointment.
One last element concerns patients with implanted cardiac devices. Modern devices are generally shielded, but checking with the cardiologist remains the appropriate course in doubtful cases.
In summary: ultrasonics remove and disrupt even where they do not reach, curettes finish and allow verification, the correct sequence uses them in order, power is kept low with the tip parallel, and an unsharpened curette burnishes instead of cutting.
