Scaler tips: why a tip worn by one millimetre works far worse

How geometry is chosen according to deposit type, why loss of length reduces effectiveness disproportionately, and the adaptation rules on root surfaces and implants.

Scaler tips are not interchangeable, and the choice follows deposit type and site. Sturdier tips with a larger cross-section are designed for abundant supragingival calculus and work at medium to high power: they remove quickly but cannot enter a pocket without traumatising it. Thin, elongated and often curved tips serve subgingival debridement, where the criterion is not force but access: they must reach the base of the pocket following root anatomy, and to do so they work at low power. Using a universal tip subgingivally means not reaching where it matters and, in trying to compensate, raising the power — which is the most direct way to damage root cementum.

The less well-known and economically most relevant fact concerns wear. An ultrasonic tip works because it vibrates at a specific frequency with an amplitude determined by its length: as it shortens through wear, the tip leaves the condition it was designed for and loses efficiency disproportionately. The literature and manufacturers' guidance converge on a surprising order of magnitude: a loss of about one millimetre entails a reduction in effectiveness of around twenty-five per cent, and two millimetres exceeds fifty. The practical result is that a worn tip makes the clinician take twice as long for the same result, and often pushes them to raise the power. The wear-check cards supplied by manufacturers exist precisely for this, and should be used: by eye the difference is not visible.

On adaptation, three rules reduce iatrogenic damage. The tip must be kept parallel to the root surface or at a minimal angle, never perpendicular: point contact concentrates energy at one spot and scores the root. Movement must be continuous and light, without pressure — pressure does not increase ultrasonic effectiveness, it reduces it, because it damps the oscillation. And irrigation must be kept on and adjusted: it cools, but above all generates the cavitation that contributes to biofilm removal.

Implants are a case apart. Metal tips scratch the titanium surface, and a scratched surface retains more plaque than before: on implant abutments and exposed surfaces, dedicated non-metallic or coated tips are needed, and this applies to routine peri-implant maintenance appointments too.
