Prophylaxis cups and brushes: the wrong hardness removes enamel instead of plaque
A hygiene session almost always ends with polishing, and it is the step where least attention is paid to instrument choice. Cup or brush is taken from the drawer without thought.
The two instruments do not do the same thing. The rubber cup works on smooth surfaces and the cervical third, where the flexible rim adapts to the contour and reaches the sulcus. The bristle brush works on occlusal fissures, where the cup slides over without entering.
Using the cup occlusally means polishing the cusps and leaving the fissure as it was; using the brush cervically means running stiff bristles over exposed cementum, which is far less resistant than enamel.
Bristle hardness should be chosen by surface, not by the tenacity of the deposit. Sound enamel tolerates medium bristles; exposed root cementum and composite require soft ones.
The difference is measurable. Cementum has far lower hardness than enamel, and aggressive polishing on an exposed root removes a thin but real layer at each session. Repeated twice a year over a decade, the effect accumulates.
Contra-angle speed matters more than the paste used. Guidance converges on low values, around 2,000 to 3,000 revolutions per minute: above that threshold friction generates heat, the paste dries and loses its role as lubricant, and abrasion increases.
The practical signal is colour. Paste that yellows or dries under the cup indicates working too fast or with excessive pressure. At correct speed the paste stays moist across the whole surface.
Pressure is the second factor: the cup should flare slightly on contact, not flatten. A flattened cup works with its rigid base instead of its rim, and loses the ability to enter the sulcus.
Paste grit should be used in sequence, not from habit. Coarse grit removes staining but leaves a rough surface retaining plaque more than the starting surface: if used, it must be followed by fine grit.
Finishing with coarse grit is a common error, producing a result that looks clean and re-stains sooner.
Single use here has a technical as well as a hygienic rationale: bristles deform after a few uses and a used cup loses rim elasticity. A reused instrument performs worse even if perfectly sterilised.
A snap-on attachment is preferable to a screw fitting for a practical reason: instruments can be changed in two seconds during the session, making it realistic to use cup and brush in the same arch rather than settling for a compromise.
In summary, prophylaxis polishing is a controlled abrasive procedure: cup on smooth surfaces, brush on fissures, soft bristles on exposed roots, low speed and fine grit last. Each of these points is trivial, and together they make the difference between cleaning and wearing away.
