How reducing filler particles to the nanometer scale has transformed composite resins, and why bulk-fill composites have changed the way deep cavities are restored.

Early composite resins were subject to significant wear, discoloration over time and a non-negligible incidence of fractures, along with episodes of secondary caries at restoration margins. The most decisive improvement in the history of these materials came from the progressive reduction in filler particle size — quartz, colloidal silica, barium- or strontium-containing silica glass, and pre-polymerized resins — and from optimizing their distribution and morphology within the resin matrix.

Latest-generation composites are mainly divided into nanofilled composites, with particles reduced to the nanometer scale that significantly improve physical and aesthetic properties, and nanohybrids, which combine these nanoscale particles with slightly larger fillers (around 0.4 micron fumed silica) to balance mechanical strength and surface polishability. A notable finding from research is that these materials' clinical performance depends not only on filler composition, but also on light-curing temperature and time: a material warmed before application has reduced viscosity and more complete monomer conversion, with better adaptation to the cavity walls.

Bulk-fill composites represent the most relevant innovation of recent years on the front of clinical practicality: unlike conventional composites, which require incremental layering in 2-millimeter thicknesses to ensure complete curing, bulk-fill materials can be applied in a single increased thickness — often up to 4-5 millimeters — thanks to greater translucency that lets light penetrate deeper and more efficient polymerization initiators. The more flowable (low viscosity) formulations still require occlusal finishing with a layer of more heavily filled conventional composite, to ensure the mechanical strength needed on chewing surfaces.

The choice among the different categories of composites available today — universal, flow, bulk-fill, high or low viscosity — depends on the specific clinical context: an aesthetic restoration in the anterior area prioritizes polishability and shade characterization, while a Class I or II restoration in the posterior area prioritizes wear resistance and application speed, a factor bulk-fill systems have made significantly more manageable than in the past.
