Endodontic Emergencies: Diagnosis and Pain Management

How to distinguish reversible from irreversible pulpitis through clinical history and diagnostic tests, and why differential diagnosis is often more complex than it seems.

Pulpitis, inflammation of the dental pulp, is among the most common reasons for urgent visits to the dental practice. According to the MSD Manual, the fundamental clinical distinction is between reversible pulpitis — in which the pulp isn't yet necrotic and the tooth can be saved with conservative treatment — and irreversible pulpitis, in which vascular damage, compromised by swelling within the rigid dentinal chamber, makes the pulp no longer salvageable, requiring root canal treatment or, in more compromised cases, extraction.

The most reliable diagnostic criterion available to the patient themselves, even before any clinical test, is the duration and pattern of pain. In reversible pulpitis, the stimulus — typically cold, heat or sweetness — causes brief pain, lasting a few seconds and resolving quickly once the stimulus is removed: the pulp is irritated but still vital and reactive. In irreversible pulpitis, pain becomes spontaneous, arising even without external stimuli, is often described as throbbing, tends to worsen when lying down due to increased blood pressure in the head, and can radiate across half the face, making it hard for the patient to precisely identify the affected tooth.

Differential diagnosis, however, is never straightforward: pulpitis and dentinal hypersensitivity can present overlapping symptoms and respond similarly to standard diagnostic tests, which is why the dentist always combines patient history with a targeted clinical exam, looking for carious cavities or leaking restorations, and with specific diagnostic tests: thermal and electric pulp vitality tests, vertical and horizontal percussion, palpation of the corresponding vestibular mucosa and, when needed, radiographic analysis. It's also important to rule out conditions that can mimic pain of pulpal origin, such as maxillary sinusitis, temporomandibular joint disorders or trigeminal neuralgia.

If pulp necrosis is left untreated, the infection can progress to form a periapical abscess, with possible spread to surrounding tissue; in more severe cases, particularly when the infection involves mandibular teeth, spread can cause severe complications such as Ludwig's angina, parapharyngeal abscess or, in extreme scenarios, systemic complications. This is why intense, persistent dental pain should never be managed with over-the-counter painkillers alone: the drug temporarily relieves the symptom but doesn't stop the underlying infectious process, which continues progressing silently until professional assessment and treatment.
