Turning the archwire end distal to the last tube is a routine manoeuvre with two precise purposes: preventing the wire from sliding forward and pricking the mucosa, and keeping archwire length stable between appointments. The Cinch Back plier exists to do exactly this intraorally, with shaped beaks that bend the end downwards without crushing it and without requiring archwire removal. The hammerhead, from which the name derives, provides the bearing surface that makes the movement controlled in a space where vision is poor and manoeuvring room minimal.

The reason a general plier will not do has to do with the material. Nitinol has shape memory and high elasticity: bent with beaks that are too sharp or over too tight a radius, it tends to notch on the tension side, and that notch becomes the point from which the wire fractures in the following weeks, often far from the chair. In practice, three precautions reduce problems. The end should first be cut with a distal end cutter that retains the offcut, so the fragment does not end up in the throat; the bend should be made in a single decisive movement, because bending, straightening and re-bending at the same point fatigues the wire far more than a single bend; and the length left distal to the tube must be enough to prevent sliding but not so much as to irritate the retromolar trigone. On maintenance, the rules of any precision instrument apply: rinsing, drying, and periodic checks of the joint and of beak closure against the light.
