Butterfly needles: why the wrong gauge ruins a draw for platelet concentrates

What a butterfly needle is for in dentistry, why too fine a gauge compromises sample quality for PRF and PRGF, and the precautions on time between draw and centrifugation.

The main use of a butterfly needle in dentistry is drawing venous blood to prepare autologous platelet concentrates, used in oral and implant surgery to support soft tissue healing and the management of post-extraction sockets. Compared with a straight needle, the winged configuration with flexible tubing extension offers two concrete advantages in a setting where venepuncture is not a daily routine: the wings allow a lower insertion angle and better control on small-calibre veins, and the extension decouples hand movements from the needle already in the vein, reducing the risk of dislodgement while changing tubes.

Gauge is the variable with the greatest bearing on sample quality, and it is where the commonest error is made. The temptation is to choose the finest gauge to reduce patient discomfort, but too narrow a lumen increases shear stress on the blood passing through it: platelets activate prematurely and haemolysis alters the sample, with the result that the concentrate obtained has characteristics different from those expected. For platelet concentrate protocols, intermediate gauges are generally preferred to very fine ones precisely to preserve cellular integrity. Flow must remain steady: a draw that struggles, with the tube filling slowly, is a draw that is damaging the sample.

The time factor is equally decisive and often underrated. In platelet-rich fibrin protocols without anticoagulant, coagulation begins the instant blood enters the tube: the interval between draw and starting the centrifuge is measured in seconds, not minutes, and a delay produces a poorly formed clot with fibrin architecture different from the intended one. Hence a practical organisational consequence: the centrifuge should be switched on, balanced and ready before the needle goes in, not after. It is the kind of detail that does not appear in the needle's technical sheet but determines the outcome of the procedure.

On operator safety, venepuncture is one of the activities with the highest risk of needlestick injury in practice. Devices with retracting or shielding safety mechanisms reduce that risk demonstrably, the needle must never be recapped, and the sharps container must be within reach at the point where the draw is performed — not across the room, which is the condition under which most incidents occur.
